Interfaith's senior services are a community effort to enable frail, ill, or disabled persons to remain in their homes and feel safe. The program recognizes that what older and disabled adults value most of all is their independence. Senior service programs provide clients with a safety net, allowing them to feel connected to their community.

Minor Home Modification

(Funded by Aging and Independent Services)
Operates throughout Inland and Coastal North San Diego County
Volunteers and local business groups are recruited to assist in making minor home repairs for qualified seniors. These repairs provide services that make it easier and safter for caregivers to manage daily caregiving activities. Services are completed by volunteers and include such repairs as faucet leaks, altering steps into ramps, repairing screen doors and windows, and installing grab bars and smoke detectors.

Project CARE (Community Action to Reach the Elderly)

Operates in the cities of San Marcos and Vista

        Case Management
Trained specialists offer assistance in working through the maze of health and social services, enabling clients to live in their own homes. Staff work to create a personalized care plan and monitor the need for health and psychological assessments.

        Friendly Visitor
Volunteers provide supportive friendship to homebound clients who have no family or friends close by and are isolated from their community. They assist with errands, letter writing, or simply spending some time with the individual.

        How R U?
This daily telephone call is made to individuals to ensure they are living safely on their own. An automated computer system makes the call at a regularly scheduled time, which is selcted by the client. If the call goes unanswered, family members and volunteers are alerted to check and see if the individual is okay.

        Vial of Life
Clients are given a small magnetic, vinyl container to place on their refrigerator door. This container holds specific information related to the individual's medical history, medications, and other health-related information. In the event of an emergency, paramedics are able to use the vial, saving precious time in providing medical assistance.

        Postal Alert
Letter carriers have teamed up to keep a watchful eye on mailboxes belonging to clients. If mail has not been picked up for two days, letter carriers will check to see if the individual is okay.

        Gatekeeper
Utility and refuse collection companies train their meter readers, refuse collectors, and account representatives to recognize and report signs that clients are in trouble.
