Senior Services

Interfaith Community Services’ Senior Services are a community effort to enable frail, ill, or disabled persons to remain in their homes in a safe and healthy environment.  The program recognizes that what older and disabled adults value most of all is their independence.  Senior Services programs provide clients with a safety net, allowing them to feel connected to their community.

Minor Home Modification

(Funded by Aging and Independent Services)
Operates throughout North San Diego County
Volunteers and local business groups are recruited to assist in making minor home repairs for qualified seniors.  These repairs make it easier and safer for caregivers to manage daily care giving activities.  Services include such repairs as leaky faucets, repairing screen doors and windows, and installing grab bars and smoke detectors.

Project CARE (Community Action to Reach the Elderly)

Operates in the cities of San Marcos and Vista
Maintains the well being and independence of seniors and disabled older adults by providing a wide range of support services including How R U daily reassurance calls, Vials of Life  emergency medical information packets, and friendly visits to homebound seniors.  The program monitors registered older adults and connects them to services and information.  These support services enable frail, isolated, and disabled older adults to remain independent and in their homes as long as possible.

Case Management

Trained specialist offers intensive hands-on assistance in working through the maze of health and social services, enabling seniors to receive services needed to remain in their own homes.  Staff work to create a personalized care plan and monitor the need for health and psychological assessments.

 

Homeless Women's Shelter

This 6 bed facility provides a safe environment where older adult homeless women with a mental illness can stabilize before moving into permanent housing.

 

Supportive Housing Programs

Two housing complexes provide permanent supportive housing for 16 older adults with disabilities.  These programs provide wrap around services to residents and work closely with case managers and mental health professionals.
